EMPLOYMENT FORMS
Complete the following and send to the NRM office (or bring to Fall DCE training) prior to doing NRM work each season.
I-9 Tax Form
Federal W-4 Tax Form
PSIA-NRM Code of Ethics for Employees and Volunteers
From Section 2.5 of the NRM Rules & Regulations: Each employee and volunteer of must sign a copy of the code of ethics and retain a copy of the code of ethics for future reference.
EVENT PAPERWORK
Complete the following paperwork for each event (please do not use existing paperwork from previous seasons, they are out-of-date).
All paperwork is to be submitted to the NRM office within 2-weeks of the event's completion. If you cannot drop it off at the office or mail it, call Turi she may be able to pick it up from Big Sky or Bridger.
- Admin Event Form - to be completed by the event clinician/examiner
- Liability Release - each participant MUST sign a release
- Evaluation Form - to be completed by participants. Print one for each person in the event.
- NRM Snowboard Exam Scorecard
- NRM Snowboard Exam Feedback Form
- New Member Application - all non-members taking a Level I exam must become members before they can receive a pin and become PSIA/AASI certified. Print one for each person in the event. Also, many KB Kids clinic attendees are non-members, so please bring a few member apps with you to that event as well.
- Expense Voucher - new for 2009-2010 season, please do not use last year's.
For all events the event clinician/examiner will be provided the following:
- a roster of registered attendees (n/a for in-house or KB events)
- for exams - pins and certificates
Since all events (apart from inhouse L1's and KB Kids) are paid for in advance the event clinician/examiner should not need to collect event fees. There is no event day registration.
