Forms for DCE Use

Event registration form - Folks should have already used this one to send in their registration, but here it is, just in case.

Event roster form - You'll need one of these to sign everyone in.

Liability release - Everyone needs to sign this one.

Evaluation form - Photocopy this one front-to-back to make it one sheet. Copy one for each person in the event. Note that an additional e-mail survey will also be sent to all exam participants.

New member application - non-members taking a Level I event need to become members in order to get the pin and become officially certified.

Expense voucher - Send this in with your receipts and mileage.

I-9 tax form - complete and send to Neil (or bring to Fall DCE training) prior to doing NRM work each season.

Federal W-4 tax form - complete and send to Neil (or bring to Fall DCE training) prior to doing NRM work each season.
